Lighthouse Care Center of Augusta has been providing psychiatric services to the CSRA for more than 20 years. Located in Augusta, GA our 84-bed facility provides a therapeutic setting for those seeking treatment for mental illness. The Partial Hospitalization Program (PHP) Therapist is a key member of the Lighthouse Clinical Services Team who will integrate and coordinate a patient centric therapeutic strategy with a keen focus on patient care; ensuring that delivery of high-quality and cost-effective treatment is consistent with the mission, vision, and values of Universal Health Services and in accordance with government regulation, licensing and accreditation requirements. The PHP Therapist is responsible for comprehensive assessment of persons whose lives are disrupted or complicated by mental illness, behavioral disturbance, substance issues or inability to function in the outpatient setting.

JOB RESPONSIBILITIES:

  • Comprehensive assessment of patients in outpatient setting.
  • Oversee individualized treatment plan, which may include nursing care, medication management, group therapy, educational support, family therapy and individual therapy.
  • Makes referrals to community resources as appropriate.
  • Coordinate discharge planning through the medical staff.
  • Function as a member of the multidisciplinary team and participate in the community relations program of the organization.
  • Participates in continuous performance improvement activities.
  • Communicates with outpatient referral sources, families/guardians, and outpatient aftercare providers.

Qualifications

Master's Degree in counseling, social work or related field.
