Description
About Gather Health
Gather Health is a Boston-area primary care practice serving adults 65 and older, many of whom face barriers to accessing care and support. We believe care is fundamentally human: the relationship between a patient and provider is foundational to helping people live well. Our value-based care model succeeds by keeping people healthy, and we build technology that gives care teams more understanding and time to strengthen those relationships.
That human relationship shapes how we build: technology should help care teams understand the whole person, act on what matters, and spend more time caring for patients.
The Role
We are looking for our first Product Designer to shape two connected products, Gather Connect and Gather Care, around one core idea: technology should deepen the connection between patients and care teams. You will own design from research and early concepts through polished interfaces and implementation, working closely with product, engineering, clinicians, operations, and the people who use what we build.
Gather Connect is our patient-facing experience between visits, delivered primarily through phone and AI-enabled conversations. It helps us understand not only what is happening with a patient’s health, but how they are feeling, what matters to them, and what may stand in the way of care. We are expanding Connect so patients can also complete tasks such as scheduling appointments, requesting medication refills, and following up on referrals.
Gather Care is our clinician-facing interface. It brings together patient conversations, clinical records, claims, hospital visits, and other information so clinicians can understand the whole person, recognize who needs what care and when, and act with confidence. Our long-term ambition is to use AI to reduce cognitive and diagnostic burden—not replace clinical judgment—so clinicians can spend more time listening to and caring for their patients.
This is a hands-on individual contributor role with broad ownership and ample opportunity for product thinking and craft.
What You’ll Work On
· Spend time with patients and care teams to understand their goals, relationships, routines, and constraints. Observe real workflows, conduct interviews, and test prototypes to learn where technology can support human connection rather than add friction.
· Turn what you learn into user flows, prototypes, detailed interactions, and polished visual design, then partner with engineering through implementation and release.
· Design Gather Connect across phone conversations, onboarding, patient preferences, and future digital touchpoints. Create AI-enabled interactions that feel natural, respectful, and trustworthy, including scripts, dialogue flows, clear handoffs to people, and recovery paths when conversations go wrong.
· Turn complex longitudinal patient information into clinical interfaces that help care teams see the whole person, not just a set of data points. Design patient summaries, prioritized needs, recommended next steps, and follow-up workflows that make it easier to provide thoughtful, timely care.
· Help clinicians use AI-generated insights without losing context or control. Make sources, timing, and uncertainty easy to understand; support quick feedback and correction; and ensure the interface reinforces clinical judgment rather than obscuring it.
· Establish reusable components and practical design conventions that help a small team move quickly without losing coherence, accessibility, or the human quality of the experience.
· Make accessibility a core part of the work. Design for older adults with varying levels of vision, memory, and comfort with technology, and create experiences that preserve dignity, clarity, and control.
What We’re Looking For
· A portfolio of shipped product work that shows end-to-end ownership—from discovery and research through launch—and demonstrates strong product thinking, craft, and impact.
· Strong interaction and visual design craft, including information architecture, user flows, typography, layout, hierarchy, and polished, production-ready interfaces.
· Ability to plan and conduct user research, test ideas with patients and care teams, and turn what you learn into practical product decisions.
· Proficiency in Figma, including high-fidelity interactive prototypes, design systems, and component libraries.
· Ability to apply accessibility standards, including WCAG, and inclusive design practices throughout the product-development process.
· At least 5 years of relevant experience required for this role
Nice to Have
· Experience with healthcare, clinical workflows, or value-based care.
· Experience designing for older adults or people with limited access to technology.
· Experience with voice, conversational interfaces, or AI-enabled products.
· Experience presenting complex data or supporting decisions in professional tools.
Please include a portfolio with examples of your product design work and a brief explanation of your role in each project.
How We Work
The Boston area is preferred because spending time with patients and care teams is central to the role. You will also collaborate closely with a distributed product and engineering team.
We value people who are direct, practical, kind, and reliable. You should enjoy getting close to a problem, making something tangible, and working with others to improve it.
Why should I join Gather Health?
· Design products that directly support patient care and stronger human relationships.
· Work with patients and clinicians to see how your decisions affect their daily lives.
· Help shape two connected products and establish design practices early.
· Join a small team with meaningful ownership and a business model built around keeping people healthy.
