RHTP Jr. Community Networks Project Manager

HCH Enterprises, LLC is seeking a Junior Rural Health Community Networks Project Manager - RHTP. This is a part-time position with HCH Enterprises, LLC, contract position with the State of Rhode Island.

General Statement of Duties: At a high level of expertise, coordinate development of rural community health networks within an assigned region of Rhode Island. This work may involve stakeholder engagement and coordination, program development and implementation, strategic planning, and research. Such responsibilities shall be executed with a high degree of independence, in support of the objectives of the Rural Health Transformation Program (RHTP) and related health, including Health Care System Planning (HCSP).

Supervision: Work under the direction and guidance of the Community Health Network Lead with ownership and of key deliverables, exercising initiative and independent judgment in the utilization of professional standards and techniques.

Supervision Exercised: Supervise personnel assigned to assist on a regular project basis and ensure that work products are developed in a qualitative and timely manner.

SCOPE OF WORK

Rural Health Transformation

Support implementation of rural community health networks in an assigned region as part of Initiative 1 of Rhode Island’s Rural Health Transformation Program (RHTP). Responsibilities shall include:

• Coordinate with RHTP community engagement team to engage stakeholders, particularly network grantees, in planning and implementation
• Work with key partners to implement rural community health networks, including developing infrastructure and tools needed for launch
• Exercise creative problem-solving to address unique challenges involved in implementing an innovative model of health infrastructure in rural Rhode Island
• Work with team members across RHTP to align projects that can impact or intersect with community health networks
• Work closely with local stakeholders to build strong relationships
• Support data collection and progress reporting
• Support activities to align RHTP objectives with HCSP (Executive Order 24-04) work
• Serve as a member of internal RHTP teams as necessary to ensure compliance with RHTP requirements and alignment with RHTP objectives

REQUIRED QUALIFICATIONS FOR APPOINTMENT

Knowledge, Skills, and Capacity:

Must have a thorough understanding of the key stakeholders in RI rural communities, including municipal government, healthcare providers, community-based organizations, resident leaders, social service directors, EMS and fire departments, advocates, and others across systems of care and populations served.

Must have significant knowledge of the principles and practices of community organizing and health program implementation, including policy and planning, program administration, research, analytics, and stakeholder engagement.

Must have excellent writing, facilitation, communication, presentation, and interpersonal skills.

Must have significant knowledge of the factors that limit access to health services and impact health outcomes of rural Rhode Islanders.

Must have a general understanding of public health, including healthcare finance, healthcare delivery, and public health objectives.

Education and Experience

Education: Such as may have been gained through: Possession of a Bachelor’s degree from a college of recognized standing with specialization in a relevant field of Social Work, Health Policy, Public Policy, Human Services, a licensed health profession, or a closely related field; and

Experience: Such as may have been gained through 2-5 years of employment in a professional position with responsibilities related to health policy, public health, public administration, program administration, or other relevant experience.

Or, any combination of education and experience that shall be substantially equivalent to the above.

This is a hybrid position, however, MUST be local. Length of contract is at least 1 year from projected start date. Annual compensation range is $63,000 - $73,000.
